Eligibility

URP Adult Day Services and URP In-Home Services clients are seniors that have cognitive and/or physical impairments who need care, socialization, Seniors that need assistance with activities of daily living in their home so that they can to remain safe during the day,

You are eligible for ADULT DAY SERVICES if you:

• Are determined physically in need of services
• A resident of the state of Illinois
• Are a United States citizen or legal alien
• Applicants must be individually assessed to determine if Adult Day Services are appropriate.
• Applicants must be capable of benefiting from socialization in a structured environment.
• Applicants must be capable of being transported to and from the facility.
• Applicants must be experiencing physical, emotional and/or cognitive difficulties that impede their ability to complete daily activities without supervision or assistance.
• Applicants must be willing to participate in a multi-disciplinary assessment of physical and social needs.
• Applicants must meet the asset requirements, of the Community Care Program (CCP) of The Illinois Department on Aging (IDOA). An IDOA Case Manager will explain asset requirements upon an initial assessment.

Services can be paid for by the participant, the participant’s family or the participant’s long term care insurance. Participants may be eligible for assistance from several public sources:

• The Illinois Department on/Aging helps low income elderly over the age of 60 who need Adult Day care Services instead of nursing home placement.
• The Illinois Department of Rehabilitation Services helps adults under the age of 60.
• The Veterans Administration helps qualified veterans.
• Daily fees for private pay ranges from $ 60.00 to $ 80.00
